Thymalin restores functional thymic tissue and promotes thymopoiesis (the production and maturation of T-lymphocytes) in aged or involuted thymic tissue. It normalises the CD4+/CD8+ T-cell ratio, restores thymic peptide secretion (thymulin, thymopoietin, thymosin), and improves T-cell receptor repertoire diversity, which narrows significantly with age. Longitudinal studies involving elderly populations demonstrated significant reductions in all-cause mortality and infectious disease burden over 6–8 year follow-up periods in groups receiving periodic Thymalin administration.

Research by Professor Khavinson's group into Thymalin dates back to the 1970s, making it one of the most extensively characterised bioregulator peptides in existence. Longitudinal studies across elderly populations provide some of the most compelling real-world anti-aging data available for any peptide compound.
The thymus undergoes progressive involution from puberty onwards, with most individuals having minimal functional thymic tissue by age 50–60. Thymalin has demonstrated the ability to restore functional thymopoiesis (the production of mature T-cells) in aged thymic tissue, increasing naïve T-cell output.
As the thymus involutes, the T-cell receptor repertoire narrows dramatically, reducing the immune system's ability to respond to novel antigens including new infections and mutated cancer cells. Thymalin treatment has been shown to partially restore T-cell receptor diversity.
A landmark longitudinal study in elderly nursing home residents showed that periodic Thymalin administration over 6–8 years reduced all-cause mortality by approximately 50% compared to control groups: one of the most dramatic mortality data points in the entire anti-aging research literature.
Thymalin is frequently combined with Epitalon in the Khavinson Institute protocols, where the two together address the two primary pillars of biological aging: immune system decline (Thymalin) and telomere shortening/neuroendocrine dysregulation (Epitalon).

Thymalin has an excellent safety record from four decades of clinical research and use across thousands of patients in Russia and Eastern Europe. Injection site reactions (mild redness or swelling) are the most commonly observed effects. No significant systemic adverse events have been documented in any of the published research. As a thymic bioregulator, it does not cause immunosuppression or autoimmune phenomena. It is well-tolerated even in elderly and immunocompromised populations: the groups in which it has been most extensively studied. The short cycle structure (10 days on, 6 months off) means cumulative exposure is inherently limited.
