SS-31 binds selectively to cardiolipin: a phospholipid found exclusively in the inner mitochondrial membrane that is essential for cristae architecture and electron transport chain function. By stabilising cardiolipin, SS-31 prevents its peroxidation by cytochrome c, restores the physical structure of mitochondrial cristae, and re-establishes efficient electron transfer through complexes I–IV. This reduces reactive oxygen species (ROS) generation, increases ATP synthesis, and lowers the metabolic cost of cellular processes. The result is improved energy output, reduced oxidative stress accumulation, and protection of cardiac, renal, and neuromuscular function.

SS-31 (Elamipretide) has been administered to humans in multiple clinical trials at doses significantly higher than typical research protocols, with a well-characterised and favourable safety profile. The most commonly observed side effects in clinical trials were transient injection site reactions (bruising, erythema, pain) in a minority of participants. Systemic side effects were rare and generally mild. No significant effects on blood pressure, heart rate, haematological parameters, or renal/hepatic function were observed at therapeutic doses. As with all injectable compounds, sterile technique is essential to avoid infection risk.
