PT-141 activates melanocortin receptors 3 and 4 (MC3R and MC4R) in the central nervous system, particularly in the hypothalamus and limbic system. MC4R activation modulates dopaminergic pathways involved in sexual arousal and motivational drive, producing a neurally-mediated increase in sexual desire and response. Because this mechanism is upstream of vascular effects, it is effective regardless of hormonal status or peripheral circulation, and works in both male and female physiology without requiring sexual stimulation to initiate its effect.

PT-141 acts directly on the brain rather than the vasculature, activating MC3R and MC4R in the hypothalamus to produce a neurally-mediated increase in sexual desire. It is the only approved pharmacological agent (as Vyleesi) indicated specifically for female hypoactive sexual desire disorder.
PT-141 is the active pharmaceutical ingredient in Vyleesi, FDA-approved in 2019 for premenopausal women with hypoactive sexual desire disorder (HSDD). This regulatory history provides an unusually strong clinical evidence base for a peptide compound.
Effects typically begin within 45–60 minutes and persist for 6–12 hours, allowing flexible timing around activity. The duration significantly exceeds that of most vascular erectile agents, providing a more relaxed and natural experience.
PT-141 does not alter testosterone, oestrogen, FSH, or LH levels. Its effects are transient and produced through receptor activation rather than hormonal change, making it appropriate for protocols where hormonal stability is important.
MC4R activation also produces mild appetite suppression as a secondary effect: relevant for users pursuing body recomposition alongside libido enhancement.

PT-141 (Bremelanotide) was approved by the FDA as Vyleesi in 2019 with a well-characterised safety profile. The most common side effects are nausea (reported in ~40% of trial participants, typically mild-moderate), flushing, headache, and injection site reactions. Nausea risk is substantially reduced by pre-dosing with an anti-emetic (e.g., ondansetron). Transient blood pressure increases (systolic +2–4 mmHg) and decreases in heart rate were observed; these are generally not clinically significant in healthy individuals but warrant caution in those with cardiovascular conditions. Hyperpigmentation of skin and gums has been reported with chronic use at higher doses (a dose- and frequency-dependent effect mediated by MC1R). Cycling is strongly recommended.
