Retatrutide operates through three complementary receptor pathways. GLP-1 receptor activation suppresses appetite, slows gastric emptying, and improves glycaemic control. GIP receptor activation enhances insulin sensitivity, improves fat metabolism in adipose tissue, and reduces the nausea side effects seen with GLP-1 monotherapy. Glucagon receptor activation increases basal metabolic rate and promotes hepatic fat oxidation: a mechanism absent in dual-agonist compounds. This triple synergy produces substantially greater fat loss than single or dual receptor approaches, preferentially targeting adipose tissue while preserving lean muscle mass, and improving lipid profiles, insulin sensitivity, and cardiovascular risk markers.

Phase 2 clinical trials demonstrated 17–24% total body weight reduction at 48 weeks: substantially exceeding the results seen with GLP-1 monotherapy or dual GIP/GLP-1 approaches. The addition of glucagon receptor agonism creates a third, additive metabolic pathway not present in semaglutide or tirzepatide.
Glucagon receptor activation increases hepatic glucose output and promotes fatty acid oxidation, raising basal metabolic rate independent of food intake. This mechanism is absent in semaglutide and tirzepatide and accounts for Retatrutide's superior recomposition profile.
The GLP-1 component suppresses appetite by acting on hypothalamic satiety centres, slows gastric emptying, and improves insulin secretion in a glucose-dependent manner: reducing post-prandial glucose spikes and insulin resistance.
GIP receptor co-activation reduces the GI side effects (nausea, vomiting) typically associated with GLP-1 monotherapy, while also improving adipose tissue insulin sensitivity and contributing to the total fat mobilisation effect.
Unlike historical weight-loss approaches, Retatrutide's phase 2 data showed that the majority of weight lost was from adipose tissue, with relative preservation of lean muscle mass: critical for maintaining metabolic rate and functional capacity during aggressive fat-loss protocols.

Retatrutide's phase 2 clinical-trial data characterises a side-effect profile broadly consistent with other incretin therapies. Gastrointestinal effects such as nausea, vomiting, diarrhoea, and constipation are the most frequently reported and are dose- and titration-rate-dependent; slow escalation significantly reduces incidence. These effects typically peak early in treatment and diminish with continued use. No serious adverse cardiovascular events were significantly elevated versus placebo. As with all GLP-1 pathway agonists, caution is warranted in individuals with a personal or family history of medullary thyroid carcinoma or MEN 2 syndrome, based on preclinical safety findings.
