Pituitary GH release drives downstream IGF-1 production, with effects on muscle protein synthesis, fat oxidation, and tissue repair comparable to other GH secretagogues. The appetite stimulation mechanism works through NPY/AgRP neuronal activation in the hypothalamic arcuate nucleus, making GHRP-6 particularly useful for protocols requiring caloric surplus alongside GH optimisation.

GHRP-6 is among the most potent GH secretagogues in terms of acute GH pulse amplitude, producing large, robust GH releases within 15–30 minutes of administration. For protocols requiring maximum GH stimulation intensity, GHRP-6 offers more pronounced GH output than selective agents like Ipamorelin.
Unlike other GHRPs, GHRP-6's ghrelin mimicry produces significant appetite stimulation through NPY/AgRP activation in the hypothalamic arcuate nucleus. This makes it uniquely valuable for users in caloric surplus phases who need both GH optimisation and caloric intake support simultaneously.
Developed in the 1980s, GHRP-6 was the first widely studied growth hormone releasing peptide and remains the reference compound against which newer secretagogues are evaluated. Its extensive research history provides a comprehensive pharmacological understanding.
As a ghrelin mimetic, GHRP-6 has the same gastroprokinetic effects as ghrelin: stimulating gastric emptying and small intestinal motility. This makes it relevant in contexts of gastroparesis or reduced GI motility, in addition to its anabolic applications.
Unlike Ipamorelin, GHRP-6 also stimulates cortisol and prolactin to some degree: a distinction that is important for protocols sensitive to stress hormone elevation. This is manageable and not clinically significant in most contexts, but informs compound selection for specific applications.
GHRP-6 combined with CJC-1295 or modified GRF 1-29 produces synergistic GH pulses that significantly exceed either compound alone. This combination is used when maximum GH output intensity is the protocol objective.

GHRP-6's side effect profile differs from cleaner selective agents like Ipamorelin. The most commonly reported effects include significant hunger/appetite stimulation (the primary GHRP-6 characteristic), mild cortisol and prolactin elevation (above Ipamorelin but generally not clinically significant at standard doses), water retention, and possible facial flushing. The hunger effect is pronounced and should be planned for in caloric-controlled protocols. Standard GH-related effects (tingling, joint discomfort) are also possible. Cycling prevents pituitary receptor desensitisation.
